These excellent doctors are getting to the bottom of a 30 year old misdiagnosis.

Tue. 10th of June 2014 I gave you the required form from the State of Florida and asked you to fill out the State of Florida form to take care of my Drivers license request. As of June 10th this year my Drivers License is no longer valid. This is my third request of you and as yet you have not had the deceancy to reply. One week from today I will send this document and all previous documents related to this failure by you to the State of Florida Medical Board in Tallahassee.
