Not a place for love ones. The service is very bad I don't know how many patients have to take care of one CNA. CNA is always in the hurry.

Lot's of screaming patients. It's too loud for my uncle.

Review Submitted Monday, December 5, 2011. My husband has been at A Grace since 9/22/2011. He had a triple bypass in July as well as a colosmy, he spent a month at San Jose Regional and a month at Kendrid Hospital.When he was transferred from Kendred Hopsital he was on a nose feeding tube, air mist from ventulator and could not move his arms or body. He is now off the feeding tube and on a regular diet, no mist, can move his arms and push himself around in the wheel chair and is now working on walking. The staff is awesome and very helpful and always has a kind word for him. I could not have picked a better facility for his recovery.

Review Submitted Thursday, September 1, 2011My husband (Ramon) was at San Jose Medical Hospital for 60 days with a TBI (traumatic brainin jury). On May 19, 2010 he had his trach tube removed. The hospital transfered him to a grace one day later. My husband still had his feeding tube and could not talk, walk and very limited movement. When we got him to a grace the staff & social worker were very professional & kind. My husband had intense therapy per my request and I facilitated this with the therapist and his doctor. All of our family visited him every day and gave him courage. The staff were awesome, I must say. Their care for my husband was in my opinion above and beyond. I could not have done this alone and am very grategul for the care my husband received. When I took my husband home on June 18, 2010 he was able to communicate with nodding to yes and no questions and he was off the feeding tube. He was able to help with his transfer from his bed to the wheelchair. My husband is now able to walk, dress himself, shower on his own, able to turn on and off the TV, and he is able to talk. I believe that the care at a grace was part of his healing and recovery. Thank you to all the staff at A Grace, may you keep up the great job your doing. I appreciate all that you have done for us!

Review Submitted Tuesday, June 28, 2011In April of this year, my mom (Susana To'omalatai) was submitted into A Grace Sub Acute and Skilled Care as a patient. Before she was submitted, she was recovering from a very high risk surgery that doctors were shocked she survived. As weeks gone by, she was gaining her strength and appetite back which allowed her to go to the next step. With a lot of research and great referrals from the nurses and social workers, we were able to hear about A Grace and how they offering everything we were wanting to provide for our mom. From the first day of my mom being submitted, she was a little nervous and scared because this type of care and living was completely foreign to her. She was coming in with her own notion that A Grace was just another hospital. But, it was in the gentle services of the rn's/cna's/rt's, the daily physical therapy, the weekly patient activities, etc...that activated new life in my mom that was lacking in her for over 12 years. When I look at her now, I no longer see a woman that was imprisoned for over 10 years with a sickness that is no longer evident in her life through the love and care of the whole staff of A Grace Sub Acute. She now appreciates life and is smiling like never before. If there is one word I could use to describe about the DNA of A Grace, it would be: Transformation. Thank you staff and social workers at A Grace for an outstanding work.

Activity Director is very rude! My uncle WAS admitted in this hospital and we planned to have a small birthday party for him and we already talk to them about it which is ok to them and suddenly the Activity Director CAME TO US just kicked us out in there telling us that there is an activity in there when they told us that the small dining hall is available that time. TOTALLY RUDE!!!! And she gave us that ANGRY FACE!

WHY I RECOMMEND A. GRACE SUB ACUTE AND SKILLED CARE



My husband Ric underwent a quadruple heart by-pass in March 11, 2010. After a month in the Intensive Care Unit at O’Connor Hospital his doctors recommended to us Kindred Hospital in San Leandro, as the best hospital for weaning patients from the respirator. After several months in that facility we were informed again that we have to find another place for him because his stay at Kindred Hospital allows him only for 35 days. A Grace Sub Acute Facility was on a list of possible places given to us to choose. That worries us because based on a rating system; A Grace is very poor, it might be another nightmare. I told my daughter that we have no choice because the facility is closed to where we live and very convenient for myself to visit him. I told my children to give a try. Ric arrived at A Grace on May 20, 2010 on a respirator. In less than a month in the facility, we were very much surprised of his improvement with the help of the capable Respiratory Therapists. Ric was out of the respirator three months and two days after his arrival at A Grace. It was a miracle indeed. I would like to give my special thanks of gratitude to all the dedicated staff, nurses and CNAs who were mostly Filipinos. We were treated as a family that even the non-filipino staff called him Tatay, Tatang, Manong, Tito or Papa. What a great feeling of comfort to know that most of the staff is indeed a family oriented to their patients. Right now Ric is continuing his physical therapy session at A Grace and without a doubt he will be going home soon. To all the Staff and Case Managers who in one way or another become an important part of Ric’s second life, thank you so much and may the Almighty God bless you all. They recently changed the name or added something to the facility name. Unfortunately, I cannot understand how the receptionist speaks; on stop of speaking really fast she alsowill not acknowledge my question. I was trying to confirm if I called the correct facility and she keeps on saying the new name of the facility and not answer my question if I was calling San Jose Sub-Acute.
