The information that we ask you to provide is not only very important to the hospital but also very important to you. Our duty as Admission Specialists is to make sure that we obtain the correct information from you for your medical records and also for billing purposes. We ask that when you come to the hospital to please be sure and bring your medical insurance cards, method of payment (cash / check / credit card), picture I.D. and if available the order from your Physician. This information along with your personal demographic information (address and phone number, etc.) will help to ensure that your time during the registration process is friendly, prompt and efficient. You may also Pre-Register for your scheduled appointments by calling our Admissions Department, Monday - Friday 8 a.m. to 4:30 p.m., at 318-254-2562. We would like for you to pre-register after the ordering physician has scheduled your appointment with the hospital.
