The South Central Rehabilitation s Hill Health Center is a federally qualified community health center established in 1968 as a collaboration between the community and the Yale Medical School. The first community health center in Connecticut, the Hill Health Center has a long history of serving New Haven neighborhoods, which are among the poorest in the state. Hill Health Center also provides health care services to the city of West Haven and towns of the Lower Naugatuck Valley, Ansonia, Derby, Seymour, Shelton, Naugatuck and Oxford. Its staff includes internists, Ob/Gyns, pediatricians, psychiatrists, psychologists, nurse practitioners, physician assistants, nurse midwives, registered nurses, certified medical technologists, certified phlebotomists, social workers, nutritionists, registered dietitians, dentists and dental hygienists. The center operates six school-based health centers and dental school-based centers. Some of center s special programs include outreach to homeless individuals and families, birth-to-three services for developmentally delayed children, school-based health centers, outreach and social/educational services to pregnant teenagers, emergency food pantry, HIV/AIDS education and outreach to at-risk populations, a 25-bed alcohol and drug detoxification program, a31-bed transitional shelter for homeless men, and a child and family guidance clinic. The Hill Health Center is located in New Haven, Conn.
