Founded in 1978, Hospice of the Western Reserve provides palliative care, caregiver support and bereavement services. It provides services in homes, hospitals, hospice houses, and assisted living and long-term care facilities. Located in Mentor, Ohio, the center offers pain management and symptom control plans and 24-hour continuous care to manage pain or acute medical symptoms. It additionally provides respite, hospice and inpatient care services. The center also offers specialized programs on heart, lungs, AIDS and pediatric care.
