Provided by YP.comThis Agency had a agent that forged my daughters name on additional policy's she did not know about and signed banking forms and medical information forms. He has been bought up on charges for 4 counts of felony fraud and additional misdemeanors. This agency is under investigation by the NY State and State Farm!!!! If you have done business with this agency and have had problems call the NY State Insurance fraud department and report.
