My mother has been in West Jacksonville Health and Rehab Center for over a year. She has Parkinsons and has gone steadily downhill. She has mysteriously suffered several injuries to her arm and a broken foot that no one knew had happened. They make her stay up all day in the wheelchair even when she is not feeling well because "she might fall out of bed". Their stories change every time you ask about things. No one wants to take responsibility for anything. She is on a pureed foods diet so they just puree everything that the other residents are eating. I tasted a bowl of beige stuff and asked what it was and it was cold, pureed bread. She has lost 16 lbs in the last 5 months. She has really gone downhill and I live in another state as does another sibling but we are searching for a better place to move her. Don't put your loved one in this place.

Very unprofessional. the lpn's are very rude especially Mrs.Gant. lpn's do not help out even if there's only two cna's for the whole buillding. you don't have a voice no matter who you voice your concern to. the place is very dirty and there's roaches everywhere. there's not enough janitors, so after a long day of work, cna's have to take the trash to the dumpster by yourself in the dark with the raccoons. Someone need to investigate the company and make some changes before it gets worst.
