Do not send your loved one to this facility. 95% of the nursing staff, admission intake person, are rude and unfriendly. The environment is not welcoming to visitors nor clients. The admission's officer is not personable when interacting with the public. My loved one fell out of the bed 3 times during the stay at the facility. I often observed staff members not being attentive to the elderly people at the facility. I observed a client sitting in the hall with vomit on her asking for assistance. Also, some clients would sit in the hallway and/or rooms for extended periods of time before someone tended to their needs and escorted them to the eating area. The nursing staff did not follow discharge instructions provided by doctors from the hospital when my loved one transitioned to the Life Center facility regarding a health matter. Due to the negligence of the staff at Life Center, my loved one had to be admitted to the hospital for several major surgical procedures

While this facility advertises hospice care as part of their services, they have billed our family for 100% of the cost of that hospice care for my mother stating that Medicare and insurance will not pay for hospice care since Medicare does not consider them a "real" hospice provider. Their billing office told me they will not even submit a claim to Medicare for this service since they know in advance it will be denied. This is an unexpected out-of-pocket expense of $3,500 for our family after the loss of my mother. When she was transferred to a hospice bed at this facility we were told that Medicare pays 100% of hospice care. Billing was sorry that we had been misinformed by the staff. We're sorry too.
