Why isn't my doctor being informed of my discomfort?

I am having problems with my medical insurance and the information Dr. Blancos assistant is giving my insurance company. She is not giving them a full report on the medications I am taking consequently insurance company dropped the Aciphex med from my policy. Ever Care wants me to take their medication(Pantoprazole Sod.) even though Dr. has told them he prefer I take acipex for my condition. It has worked for me for a few years and the new one Ever Care wants me to try is not workin..causing me problems. I believe if the Dr's office and the insurance get my record straight I would not be going through all this stress, which is not helping my stomach situation.
