Some of the staff are helpful and caring , but one gentleman was rude.

I am infuriated at the treatment I received in your Boca Raton store. I went outside for a phone call and when I came back in I was asked to re-fill out my my medical history questionnaire (which was huge, containing complete family medical history). I said that I had already done that. The woman said that my medical paperwork was shredded when I walked out to make a phone call. The manager was extremely rude when I objected and told me not to come back - twice, a second time as I was leaving.

Disgusted with your behavior. Of course, I'll never go back.
