I had a friend stay at the Canyon Transitional Rehab in April 2014. We were greeted with the odor of urine and feces on more than one occasion. The furniture and carpets were soiled but the staff acted like this was the norm. They need a new chef as the food is terrible and the patients deserve better. This place is 50% nursing home. My friend needed assistance to the restroom. She pressed the call button and waited and waited, no one came so she had to wet the bed. They are very short staffed. This place proudly displays a sign boosting they won the 2011 Bronze Commitment Quality Natl Award. This is puzzling compared to the lack of professional environment I witnessed day after day. This place should be reported to the Dept of Health for their failure to provide medical assistance to their patients. They clients are paying good money for this sub standard service. What a sad and depressing place, our elderly deserve better!

I had my grandmother in this facilty in Feb 2014. CNA's, Dietary staff, Nursing very helpful one specific one I believe it was the Staffing person very nice lady. Hellped whenever i needed something. .Business office was also great those ladies helped me step by step with the Medicaid Process. On another note staffing was short, and from what I heard it was low census I was very disappointed with the Administrator. I remember talking with an employee about my grandmother and she interrupeted with no apologies. That blew me away! Talk about BAD customer service. I saw a huge turn over between the time my grandmother was at this facility till she left, mostly managment. Sad. Just thought i would give my input so families will know what to expect. This place is under-staffed and the administration sorely wanting. The care is minimal especially as a rehabilitation center. It was and remains a nursing home despite their attempts to charge Medicare and insurance companies more by claiming to have skilled nursing and actual rehab. The rehab received here was minimal and of little use. 15 minutes of generally under-attentive care, the rehab was at most times useless and at worst, dangerous. Unable to walk a patient was left unattended to pedal a machine while the "therapists" were (as always) busy on either their cell phones or talking to one another. A patient fell out of bed and went unattended for nearly 2 hours. "Help" only came when that person's roommate called her own home to let someone know. They then had to call the nurse's station. There are no phones or other communications in the rooms other than an antiquated call light system (to which no one then sees or responds). Very poor excuse for a rehab and the administration was unhelpful, patronizing and inept.

Thanks to the staff of Canyon Transitional!! My father was there for two weeks to regain his strength after an episode in the hospital. The entire staff has been wonderful to work with from the time he was admitted throughout his entire stay. After looking at many facilities, I found that this one was clean, repective of the patients, and cheery - which makes all the difference when you are recouperating. I HIGHLY recommend Canyon Rehab center for anyone who is looking for a caring and proffessional place for themselves or a loved one. With their help and caring, Dad is able to go back home and be independent once more!! Kudos to the staff and therapist !! Thank you for making this all possible!!

Very disappointed with this facility. Short staffed and not concerned about the patients well being. My family member contracted an infection while in this facility, which led to more health problems and complications. Other facilities offer better care, hygiene and medical attention for their patients. This facility just wants the money that is given to it by an individuals health plan, but does not provide the care that it should.

Caring staff and excellent therapy

My experience at Canyon was wonderful. The staff was very patient and always answered my many questions, They explained what the goals were in order for my mom to be able to come home and helped her to be successful. The nurses were caring and greeted me every evening when I walked in with a report of how my mom was doing. I felt like part of the team!

My loved one was at the facilty and the Adminstrator and the rehabilitation department were superb. I got the feeling that Skilled healthcare the company that owns the property isn't concerned about there patients or residents as they refer to them. We always noticed everyone running to get their jobs done. The company needs to pay for more staff. It's too bad I would have kept Dad there but I just felt like the staff was overworked, underpaid and no support from their corporate offices.Dad is in an assisted living and doing well thanks to them. I just hope that they are appreciated.
